Medical Insurance Requirements
The Milligan University Sports Medicine Department is committed to providing quality health care to its student-athletes. While most care is obtained on-campus by the athletic training staff, certain injuries require a more in-depth evaluation by a physician. When off-campus care is needed or unavailable on campus, health care cost and responsibilities become a factor.
Milligan University (MU) carries a secondary insurance policy designed to help cover costs incurred because of injuries sustained during supervised practices, contests, or travel that are defined by the policy. This insurance is an “excess” insurance coverage and is never guaranteed.
To be considered eligible for this insurance policy each student athlete must complete annual Milligan University Sport Medicine participation forms on Healthy Roster. These include forms related to student – athlete demographics, parent information, primary/secondary insurance information, HIPPA Protected Health Information, NAIA substance abuse forms, waiver of claims & acknowledgement of responsibility, medical history, medical health questionnaire – PHQ-9 and a detailed pre-participation examination.
If any of these forms are not completed to acceptable parameters, the student athlete is not medically eligible for participation at Milligan University and will not be covered under the excess insurance policy. All student-athletes must also have a valid primary medical insurance carrier that will cover intercollegiate athletic injuries, and a current copy of the active insurance card is on file on your Healthy Roster account.
A primary insurance carrier is REQUIRED for each student-athlete. If you do not have a primary health insurance policy, you must do so. If a commercial carrier (i.e. Anthem, United HealthCare, Cigna, Humana, Medicaid, Spirit) is not available, please contact the MUSM athletic trainers immediately to discuss options. If the student-athlete is covered by a commercial carrier, a current copy of the active insurance card is required.
If your primary insurance company requires you to see a specific Physician (HMO, Medicaid, etc.…) for all injuries and illnesses, please attempt to change this to a local physician, if possible. This will make it easier and cheaper to get your child seen in case of unforeseen injuries and illnesses that can occur during intercollegiate athletics while at Milligan University. If your child has TennCare, please indicate your child’s assigned primary care physician in the Tri-cities, TN/VA area.
Payment of athletic related medical expenses requires coordination and cooperation of the student-athlete, parent(s), and/or guardian(s), primary and/or secondary insurance carriers, medical providers, and athletic insurance policy company. The ongoing responsibility and cooperation of the student-athlete and their families is essential and expected.
